f. Theauthorized routingover City streets including the origin and termination point
within theCity.
g. The feepaid.
h. Thedateand signature of theChief of Police or his or her designee.
i. Inaddition, thepermit will specifygeneral conditions that the permitteemust comply
with that are consistent and reasonable for theprotectionof the general public and
City streets.
2.
It is theduty of the permittee to readand familiarize himself or herself with the permit
provisions upon receipt. Undertaking of thepermitmove is deemedprima facie evidence
of acceptance of the permit and that:
a. Thepermittee is in compliancewithall operational and provision requirements;
and
b. All dimension, axle spacing andweight limitations specified in thepermit will not be
exceeded; and
1. the spacing between the steer axle and the first axle on the
tractor tandemmaynot be less than 8 feet 1 inch; and
2. the spacing between the last axleon the tractor and the first
axleof the trailermaynot be less than 18 feet 6 inches; and
3. the sum of all axle spacing’smust beat least 43 feet 6
inches
c. All operation, registrationand license requirements have been compliedwith; and
d. All financial responsibilities, obligations andother legal requirements have beenmet;
and
e. Thepermittee assumes all responsibility for injury or damage topersons or to public
or private property, including his or her own, or to the object being transported,
causeddirectlyor indirectly by the transportationormovement of vehicles and
objects authorized under the permit. He or sheagrees tohold theCity harmless from
all suits, claims, damages, or proceedings of any kind and to indemnify theCity for
any claim itmaybe required to payarising from themovement.

Thepermit shall be carried in the vehicle (either physically or electronically) towhich the
permit applies at all timeswhileoperating on streetswithin theCityand shall be exhibited
upon demand to any lawenforcement officer, police officer or authorized official of the
City.
4.
Whenever any vehicle is operated in violation of the provisions of aCitypermit whether it
beby size, weight or general/special provisions, and either or both the owner or driver of
such vehicle shall be deemedguilty and either or both theowner or the driver of such
vehiclemaybeprosecuted for such violation.
